Mechanism of action: Alendronate inhibits osteoclast-mediated bone resorption, leading to an increase in bone mass and a reduction in the incidence of fractures.
Drug usage cases: Alendronate is primarily used for the treatment and prevention of osteoporosis in postmenopausal women and men. It is also used to increase bone mass in men with osteoporosis and for the treatment of osteoporosis induced by glucocorticoid treatment. Additionally, it is employed in the management of Paget’s disease of bone.
Drug contra indications: Contraindicated in patients with abnormalities of the esophagus which delay esophageal emptying, such as stricture or achalasia; inability to stand or sit upright for at least 30 minutes; hypocalcemia; and hypersensitivity to alendronate or any of its components.
Side effects: Common side effects include gastrointestinal issues such as abdominal pain, dyspepsia, constipation, diarrhea, or flatulence. Rarely, it may cause esophageal ulcers, osteonecrosis of the jaw, and atypical femoral fractures.
Warnings: It is important to maintain adequate intake of calcium and vitamin D during treatment. Patients should be informed about the importance of adhering to the dosing instructions, especially the recommendation to stand or sit upright for at least 30 minutes after taking the medication to reduce the risk of esophageal irritation.
Use during pregnancy or breastfeeding: Alendronate is not recommended for use in pregnant women due to the lack of relevant studies. It is unknown if alendronate is excreted in human breast milk; therefore, caution should be exercised when alendronate is administered to breastfeeding women.
