Generic name: Aspirin and dipyridamole
Drug class: Platelet aggregation inhibitors
Dosage form: Oral capsule, extended release (25 mg-200 mg)
Route of administration: Oral
Dose: 25 mg of aspirin and 200 mg of dipyridamole, taken orally twice daily
Mechanism of action: Aspirin inhibits platelet aggregation by irreversibly inhibiting the COX-1 enzyme, thereby reducing the formation of thromboxane A2, a platelet activator. Dipyridamole inhibits platelet activation by inhibiting the uptake of adenosine into erythrocytes and endothelial cells, leading to increased blood flow to the heart.
Drug usage cases:
- Secondary prevention of stroke in patients who have had transient ischemia of the brain or completed ischemic stroke due to thrombosis
Drug contraindications:
- Hypersensitivity to aspirin, dipyridamole, or other NSAIDs
- Asthma with nasal polyps and rhinitis
- Active bleeding disorders
- History of gastrointestinal bleeding or ulcers
- Severe liver or kidney impairment
- Children and teenagers with viral infections, due to the risk of Reye’s syndrome
Side effects:
- Headache
- Dizziness
- Abdominal pain
- Nausea
- Diarrhea
- Bleeding complications, such as gastrointestinal bleeding
- Hypotension (low blood pressure)
- Elevated liver enzymes
Warnings:
- Increased risk of bleeding; monitor for signs of bleeding and seek medical attention if bleeding occurs
- Use with caution in patients with a history of gastrointestinal ulcers or bleeding
- Discontinue use prior to elective surgery to reduce bleeding risk
- Monitor liver function during treatment
- Use with caution in patients with low blood pressure
- Not recommended for use in children and teenagers with viral infections due to the risk of Reye’s syndrome
Use during pregnancy or breastfeeding:
Aspirin and dipyridamole should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not recommended during the third trimester due to the risk of bleeding. The safety of this combination during breastfeeding is not well established; consult a healthcare provider before use.
