Mechanism of Action: Gemfibrozil works by reducing the production of triglycerides and very-low-density lipoprotein (VLDL) in the liver. It also helps to increase the breakdown of lipoprotein particles and can increase the level of high-density lipoprotein (HDL).
Drug Usage Cases: Gemfibrozil is used to treat very high cholesterol and triglyceride levels in people with pancreatitis and to prevent the risk of developing pancreatitis in patients with very high blood fats who have not responded to other treatment methods.
Drug Contraindications: Contraindicated in patients with liver or kidney disease, gallbladder disease, and in those who are also taking repaglinide.
Side Effects: Common side effects include stomach pain, constipation, and headaches. Serious side effects can include liver problems, severe anemia, and muscle pain or weakness.
Warnings: Gemfibrozil should be used with caution in patients who are also taking statins due to an increased risk of muscle toxicity. Regular monitoring of liver function and blood lipid levels is recommended.
Use During Pregnancy or Breastfeeding: Gemfibrozil is classified under FDA Pregnancy Category C. Animal studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks. Caution should be exercised when administering to a nursing mother due to potential excretion in human milk.
