Generic name: lamotrigine
Drug class: triazine anticonvulsants
Dosage form: chewable dispersible tablets
Route of administration: oral
Dose: 5 mg, 25 mg
Mechanism of action: Lamotrigine stabilizes neuronal membranes by inhibiting voltage-sensitive sodium channels, thereby reducing the release of excitatory neurotransmitters and controlling seizure activity.
Drug usage cases:
- Seizure disorders (e.g., partial seizures, tonic-clonic seizures, Lennox-Gastaut syndrome)
- Bipolar disorder (maintenance treatment to delay mood episodes)
Drug contraindications:
- Hypersensitivity to lamotrigine or any component of the formulation
- Concurrent use with dofetilide
Side effects:
- Serious skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis)
- Hemophagocytic lymphohistiocytosis (HLH)
- Aseptic meningitis
- Suicidal thoughts and behaviors
- Rash
- Dizziness
- Headache
- Nausea
- Vomiting
- Double vision
- Blurred vision
- Loss of coordination
Warnings:
- Risk of serious skin reactions; monitor for signs of rash, especially during the first 2 to 8 weeks of therapy
- Potential for HLH; seek immediate medical attention if symptoms such as fever, enlarged liver, swollen lymph nodes, skin rashes, yellowing of skin or eyes, unusual bleeding, or neurological issues occur
- Risk of aseptic meningitis; contact healthcare provider if experiencing headache, fever, stiff neck, nausea, vomiting, rash, or sensitivity to light
- May cause dizziness or drowsiness; use caution when operating vehicles or machinery
- Gradual dose escalation recommended to minimize the risk of adverse reactions
Use during pregnancy or breastfeeding: Lamotrigine is classified as a Category C drug during pregnancy, indicating that risk cannot be ruled out. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Lamotrigine is excreted in breast milk; caution is advised when administering to nursing mothers. Consult a healthcare provider to discuss potential risks and benefits.
