Generic name: Ethinyl Estradiol, Norethindrone Acetate, and Ferrous Fumarate
Drug class: Contraceptives
Dosage form: Oral tablets
Route of administration: Oral
Dose: 1 mg of Norethindrone Acetate, 20 mcg of Ethinyl Estradiol, and 75 mg of Ferrous Fumarate daily for 24 days, followed by 4 days of placebo tablets containing 75 mg of Ferrous Fumarate
Mechanism of action: Ethinyl Estradiol and Norethindrone Acetate are synthetic hormones that prevent ovulation, alter cervical mucus to impede sperm penetration, and modify the endometrial lining to prevent implantation. Ferrous Fumarate provides iron supplementation to compensate for menstrual blood loss.
Drug usage cases:
- Prevention of pregnancy
- Management of moderate acne in females aged 15 and older
- Regulation of menstrual cycles
- Reduction of menstrual blood loss
- Management of premenstrual dysphoric disorder (PMDD)
Drug contraindications:
- Known or suspected pregnancy
- Uncontrolled hypertension
- History of thromboembolic disorders (e.g., deep vein thrombosis, pulmonary embolism)
- History of stroke or myocardial infarction
- Known or suspected breast cancer or other estrogen-dependent neoplasms
- Active or history of liver disease (e.g., cirrhosis, liver tumors)
- Undiagnosed abnormal genital bleeding
- Hypersensitivity to any component of the formulation
- Concurrent use of certain medications that may interact adversely (e.g., some anticonvulsants, rifampin)
Side effects:
- Headache
- Nausea and vomiting
- Breast tenderness
- Breakthrough bleeding or spotting
- Weight changes
- Mood changes, including depression
- Decreased libido
- Elevated blood pressure
- Increased risk of thromboembolic events
- Jaundice
Warnings:
- Smoking increases the risk of serious cardiovascular events, especially in women over 35 years old
- Monitor blood pressure regularly; discontinue if sustained hypertension develops
- Assess for signs of thromboembolic events; discontinue if suspected
- Evaluate for signs of depression; discontinue if severe depression occurs
- Monitor for signs of liver dysfunction; discontinue if jaundice or other hepatic symptoms develop
- Use caution in women with a history of gallbladder disease
- Not recommended for use during pregnancy; discontinue if pregnancy is suspected
- Not recommended for use during breastfeeding; may affect milk production
Use during pregnancy or breastfeeding:
Microgestin 24 FE is contraindicated during pregnancy. If pregnancy occurs during use, discontinue the medication and consult a healthcare provider. The safety of Microgestin 24 FE during breastfeeding has not been established. It is not recommended for use in lactating women due to potential effects on milk production and composition. Alternative contraceptive methods should be considered during breastfeeding.
