Generic name: opicapone
Drug class: Dopaminergic antiparkinsonism agents
Dosage form: Capsules
Root of administration: Oral
Dose: 50 mg orally once a day at bedtime
Mechanism of action: Opicapone is a selective and reversible inhibitor of catechol-O-methyltransferase (COMT), an enzyme that metabolizes levodopa. By inhibiting COMT, opicapone increases the availability of levodopa to the brain, thereby reducing “off” episodes in patients with Parkinson’s disease.
Drug usage cases:
- Adjunctive treatment to levodopa/carbidopa in patients with Parkinson’s disease experiencing “off” episodes
Drug contraindications:
- Concomitant use of non-selective monoamine oxidase (MAO) inhibitors
- Pheochromocytoma
- Paraganglioma
Side effects:
- Dyskinesia
- Dizziness
- Constipation
- Dry mouth
- Hallucinations
- Impulse control disorders (e.g., gambling, binge eating, compulsive shopping)
- Orthostatic hypotension
- Weight loss
Warnings:
- May cause sudden sleep episodes during daily activities
- Monitor blood pressure regularly
- Use caution when driving or operating machinery until effects are known
- Discontinue if neuroleptic malignant syndrome or rhabdomyolysis occurs
- Monitor for signs of impulse control disorders
