Generic name: icosapent ethyl
Drug class: Omega-3 fatty acid ethyl ester
Dosage form: Capsule, liquid-filled
Root of administration: Oral
Dose: 4 grams per day, taken in two divided doses with food; either four 0.5-gram capsules twice daily or two 1-gram capsules twice daily
Mechanism of action: Icosapent ethyl is a purified form of eicosapentaenoic acid (EPA), an omega-3 fatty acid. It reduces triglyceride levels and has anti-inflammatory effects, which may contribute to cardiovascular risk reduction.
Drug usage cases:
- Adjunct to maximally tolerated statin therapy to reduce the risk of heart attack, stroke, coronary revascularization, and unstable angina requiring hospitalization in adults with elevated triglyceride levels (≥150 mg/dL) and established cardiovascular disease or diabetes mellitus with two or more additional risk factors for cardiovascular disease
- Adjunct to diet to reduce triglyceride levels in adult patients with severe hypertriglyceridemia (≥500 mg/dL)
Drug contraindications:
- Hypersensitivity to icosapent ethyl or any component of the formulation
Side effects:
- Serious bleeding events, especially in patients on anticoagulant or antiplatelet therapy
- Heart rhythm abnormalities, including atrial fibrillation and atrial flutter
- Allergic reactions, particularly in individuals allergic to fish or shellfish
- Musculoskeletal pain
- Peripheral edema (swelling of the hands, legs, or feet)
- Constipation
- Gout
Warnings:
- Monitor for bleeding risk, especially in patients on anticoagulant or antiplatelet therapy
- Assess for heart rhythm abnormalities, particularly in patients with a history of arrhythmias
- Discontinue use if signs of allergic reactions occur
- Monitor liver function during treatment
