Generic name: vonoprazan
Drug class: Potassium-competitive acid blocker (PCAB)
Dosage form: Tablets
Route of administration: Oral
Dose: For Erosive Esophagitis and Non-Erosive GERD: 10 mg once daily. For Helicobacter pylori infection: 20 mg twice daily for 14 days, in combination with amoxicillin and clarithromycin or amoxicillin alone.
Mechanism of action: Vonoprazan inhibits gastric acid secretion by competitively blocking the potassium-binding site of the gastric H+/K+ ATPase pump, leading to increased gastric pH.
Drug usage cases:
- Healing of all grades of Erosive Esophagitis
- Maintenance of healing of all grades of Erosive Esophagitis
- Relief of heartburn associated with Erosive Esophagitis
- Relief of heartburn associated with Non-Erosive GERD
- Treatment of Helicobacter pylori infection in combination with amoxicillin and clarithromycin (Triple Pak) or amoxicillin alone (Dual Pak)
Drug contraindications:
- Hypersensitivity to vonoprazan or any component of the formulation
- Concurrent use with rilpivirine-containing products
- Known hypersensitivity to amoxicillin or other beta-lactam antibiotics
Side effects:
- Diarrhea
- Temporary changes in sense of taste
- Vaginal yeast infection
- Stomach pain
- Headache
- High blood pressure
- Cold-like symptoms
- Irregular heartbeats (with Triple Pak due to clarithromycin)
- Liver problems (with Triple Pak due to clarithromycin)
- Severe allergic reactions (e.g., anaphylaxis)
- Severe skin reactions (e.g., Stevens-Johnson Syndrome, Toxic Epidermal Necrolysis)
- Antibiotic-associated diarrhea (including Clostridioides difficile-associated diarrhea)
Warnings:
- QT prolongation risk (with Triple Pak due to clarithromycin)
- Hepatotoxicity (with Triple Pak due to clarithromycin)
- Serious drug interactions (e.g., with colchicine, warfarin, benzodiazepines)
- Embryo-fetal toxicity (not recommended during pregnancy)
- Exacerbation of myasthenia gravis (with Triple Pak due to clarithromycin)
- Risk of Clostridioides difficile-associated diarrhea
Use during pregnancy or breastfeeding: Vonoprazan is not recommended during pregnancy due to potential risks to the fetus. It is not known if vonoprazan passes into breast milk; caution should be exercised when administering to nursing mothers.
